So "She often visits her grandmother on Sundays." is correct. In "She visits often her grandmother on Sundays." the adverb separates the verb from its direct object. In "Often she visits on Sundays her grandmother." the adverbial of time comes between the verb and the object. In "She visits her often grandmother on Sundays." the adverb stands inside the noun phrase and changes the meaning.
